  • Deductibles for comprehensive and collision (not exceeding $1,000 as set by JSC FCU)
  • Credit Union listed as loss payee/lien holder as follows:

  • Correct collateral description, including:
    • Year
    • Make
    • Model
    • Complete VIN
  • Borrower/Insured name
  • Insurance company name
  • Policy number
  • Coverage effective/renewal date

How to Submit Your Proof of Insurance?
